Job Summary:

The Medical Transcriptionist will be responsible for providing quality patient care in a prompt and professional manner. 

Job Description:

The Medical Transcriptionist will perform all or some of the following duties & responsibilities:

  • Transcribe medical information using a computer, voice processor and applicable software

  • Apply knowledge of medical terminology, anatomy and physiology, disease processes, signs and symptoms, medications, allergy interactions and laboratory values

  • Apply knowledge of medical transcription guidelines and practices per AAMT guidelines

  • Operate word processing equipment, dictation and transcription equipment and software as specified and show the skills to troubleshoot problems as necessary

  • Apply relevant legal concepts per confidentiality, privacy and information security guidelines

  • Apply knowledge of ADT information to the patient record as relevant to ensure proper placement within the electronic chart

  • Perform other duties required.   

Minimum Requirements / Knowledge / Skill For Medical Transcriptionist Job

  • One or more years experience in medical transcription in a physician’s office or as general hospital transcriptionist

  • Possess 2 or more years experience in medical transcription in a general hospital transcription setting or with a medical transcription service serving a general hospital preferred

  • Possess more extensive experience than the minimum of Radiology services

  • Possess extensive knowledge of medical terminology, anatomy, physiology, laboratory, pathology terminology, surgical instrumentation and pharmacology

  • Strong recent background in operative notes strongly preferred

  • Experience with the 4 major work types preferred

  • Possess the ability to reach and maintain a 98% accuracy

  • Possess experience working with English-as-a-Second-Language (ESL) dictators

  • Access to high-speed Internet (DSL, cable modem, wireless, or satellite) service.

  • High School diploma or equivalent GED

  • Certified Medical Transcriptionist CMT from AAMT preferred.

Job Summary:

The Medical Transcriptionist - Transcription Services, under general supervision will be responsible for transcribing a variety of medical reports, notes and other data from recorded dictation. 

Job Description:

The Medical Transcriptionist - Transcription Services will perform all or some of the following duties & responsibilities:

  • Transcribe recorded material, including operative reports, discharge summaries, patient history and examinations

  • Prioritize current dictations by chronology or urgency. Log dictation to avoid lost or missing reports

  • Verify accuracy of patient information such as name, hospital number, location in hospital and patient file location

  • Make entries on prescribed control forms as needed, indicating report that was transcribed, dates dictation was received and transcribed, and name of dictator

  • Maintain a production report detailing numbers and variety of reports completed per day, as well as line count for each day

  • Dispatch finished reports to designated persons for approval and signature

  • Duplicate documents and forms as directed and mail to outside individuals and agencies

  • Perform other duties that may be assigned.

Example 3: Medical Transcription Supervisor Job Description

Job Summary:

The Medical Transcription Supervisor will be responsible for managing the day-to-day operations and activities of the transcription section. He/She will monitor work volumes and give assignments based on priorities and staffing.

The Medical Transcription Supervisor will monitor daily productivity and work quality of the transcriptionists; address staffing issues and corrective action plans; provide statistics and report on a routine basis and as required.

Administratively the Medical Transcription Supervisor will maintain and support the dictation and transcription system; facilitate correction of dictations and transcriptions; and support the overall transcription needs of the local medical centers.  

Job Description:

The Medical Transcription Supervisor will perform all or some of the following duties & responsibilities:

  • Supervise on-site and remote (work-at-home) staff and the timely and accurate transcription of physician dictations

  • Monitor the daily work volume and assign work accordingly by work type and priority

  • Monitor daily productivity of the transcriptionists and perform quality review of work performed by the transcription staff; including analyzing results and addressing issues

  • Compile and submit reports and statistics on a regular basis and as requested by management

  • Monitor the uploading of documents from the transcription system into HC and resolving any interface errors or issues

  • Perform administration of the dictation and transcription system working closely with IT on the maintenance of the system and addressing any issues

  • Ensure compliance with federal, state and local regulations as it pertains to quality of transcribed documents

  • Consistently support compliance and the Principles of Responsibility (code of conduct) by maintaining confidentiality, protecting the assets of the organization, acting with ethics and integrity

  • Perform other duties required.

Minimum Requirements / Knowledge / Skill For Medical Transcription Supervisor Job

  • Possess three (3) or more years work experience in a health care, health information management or business office setting
  • Possess knowledge of federal and state regulations, as well as Joint Commission requirements in HIM
  • Proficient in using a computer, email system, and Microsoft Office Suite (Excel, Word, and Powerpoint)
  • Possess experience preparing statistics and reports
  • 2 or more years experience in managing transcription. 

Minimum Education Requirements / Certification / Licensure For Medical Transcription Supervisor Job

  • Associate's Degree (AA/AS) in Health Information Management or other health care related fields
  • Bachelor's Degree (BA/BS) in Health Care related field is preferred
  • RHIT(Registered Health Information Technician), or RHIA (Registered Health Information Administrator) certification by AHIMA
  • 5 or more years experience supervisory experience in HIM in lieu of certification.

Example 4: Medical Transcription Vendor Clerk Job Description

Job Summary:

The Medical Transcription Vendor Clerk will be responsible for monitoring outsourced transcription service activities to ensure contractual service levels are met. He/She will review invoices for accurate billing and payment of services. 

Job Description:

The Medical Transcription Vendor Clerk will perform all or some of the following duties & responsibilities:

  • Assist the Transcription Manager or Lead in allocating work to the vendor for completion

  • Assist with daily NMI interface monitoring which includes resolving any errors or rejections to MEDITECH Order Entry reports crossing the gateway

  • Correct report demographics as needed

  • Troubleshoot report distribution processes as needed

  • Work MEDITECH and other system interface report rejection queues and edit routines as required

  • Respond to requests and other inquiries from customers

  • Perform searches for reports not received from external vendors but that may have been dictated

  • Run statistical and management reports as needed and aggregate performance statistics

  • Assist with the completion or routing of work assignments within the transcription work queues as required

  • Assist with the identification and or remediation of report issues such as cancelled, amended, or edited reports to ensure the integrity of the legal health record

  • Promptly report problems with transcription vendor services or equipment to Transcription Manager

  • Perform other duties as assigned.
