Job Summary:

The Nurse Case Manager - On-call is responsible for hospital care management.

Job Description:

The Nurse Case Manager - On-call will perform all or some of the following Job description:

  • Perform all programs of the health care system within an assigned Clinical Program, functioning with collaborative implementation and evaluation of Clinical Integration Programs, Care Process Models, Care Management, Utilization Management, and Patient Teaching.

Minimum Requirements / Knowledge / Skill For Nurse Case Manager - On-call Job

  • Three years of clinical hospital experience

  • Ability to communicate successfully with all levels of individuals and groups both professional and private

  • Highly self-motivated with a positive attitude and flexibility in a rapidly changing environment

  • Excellent ability for conceptual thinking, good listening, problem resolution, and planning skills

  • Proven management and leadership skills

  • Computer skills.
     

Minimum Education Requirements / Certification / Licensure For Nurse Case Manager - On-call Job

  • Graduation from an accredited school of nursing, BSN

  • Current State RN licensure.
