The Nurse Health Partner is
responsible for complete care management
of members needs and coordinate services
with providers.

The
Nurse Health Partner will perform
all or some of the following Job
description:

Collaborate with Medical Doctors in
delivering care to patients

Responsible for adequately
addressing the progression of the
patients illness and the
corresponding home care services
required
-
Coordinate provisions of all
ancillary services as well as health
care services not included within
the Company benefit package
-
Travel to patient homes is required
-
Participate in educational programs
and professional committees.
Minimum Requirements / Knowledge / Skill
