Nurse Practitioner will perform
all or some of the following Job

Provide direct care to aging
residents in long-term care
facilities

Collaborate with primary care
physicians, facility staff and
patients' families

Participate in care conferences with
other providers and patient family
members

Work
with the facilities utilization
review process

Identify patients' behavioral health
issues

Educate nursing home staff,
physicians and administration
-
Coordinate care for patients.
Minimum Requirements / Knowledge / Skill

Clinical experience

Long-term care experience

Knowledge of mental health issues

A
background in managed care

Familiarity with Medicare and ICD-9
coding.
