The Staff Field Nurse will perform all or some of the following Job description:

• Provide home care visits and assume on-going primary responsibility for an assigned caseload of patients within a geographical area

• Responsible for conducting initial home assessments and evaluations of patients nursing and healthcare needs

• Determine appropriate billing source for patients based on documentation from home care visits

• Coordinate activities with other health team members to ensure optimal patient care and communication of pertinent findings.

• One-year acute, medical-surgical nursing experience

• CHHA/LTHHCP experience preferred

• Excellent clinical and assessment skills with the ability to work within an integrated health care team

• Effective critical thinking skills and the ability to solve problem independently

• Excellent verbal and written English skills.

• Associates/Diploma from an accredited school of Nursing; Bachelors Degree preferred
• Licensed with current Registration to practice as Registered Professional Nurse.
